Two application pathways exist: Form 2 for applicants with qualifications in Appendix B (simplified assessment) and Form 3 for all other applicants (comprehensive assessment). Applicants are responsible for the accuracy of their application. False or misleading information may result in declined assessment, future applications being declined, and (for Skilled Migration applicants) notification to the Department of Home Affairs. Eligibility Criteria
6 criteria- Hold an overseas optometry qualification
- Be a qualified optometrist seeking to practice in Australia and/or New Zealand
- Applicants with qualifications listed in Appendix B may use Form 2 (simplified process)
- Applicants without qualifications in Appendix B must use Form 3 (full assessment)
- Current registration as an optometrist required
- Hold a valid passport
- Current and valid passport page showing photograph and passport signature (600dpi colour scan)
- Colour passport-sized photograph taken within last 12 months (35-40mm wide, 45-50mm long)
- Evidence of name change if applicable (e.g. marriage certificate)
- Degree/qualification certificate (Form 2) or Certificate of qualifications in Optometry from issuing institution(s) (Form 3)
- Official academic transcript (Form 3 only)
- Evidence of supervised clinical training - at least one equivalent full-time academic year as clinical training with direct patient contact and independent management decisions reviewed under supervision (Form 3 only)
- Certificates from all authorities with which you are registered as an Optometrist
- Course handbook/bulletin/official syllabus showing topics covered, instructional hours breakdown, teaching weeks per semester, and prescribed textbooks (Form 3 only, minimum 300dpi)
